school purchase order
Will you accept a public school purchase order for a purchase of you safety glasses? These are being purchased for one of our students. Please let me know as soon as possible so we can get this processed.

We do need completed form to allow invoicing of purchase orders. I am forwarding it to you as a PDF file, it can be electronically completed. Your account will be opened as soon as it is completed and then we can accept your purchase order.
